Dr. Shane Tolleson is a UHCOP alum, from the class of 2014. After graduating with his PharmD, Shane went on to complete a PGY1 in Pharmacy Practice at Baylor St. Luke’s in the Texas Medical Center. He then completed 5 months of training as a PGY2 in Solid Organ Transplant, followed by a transition to a Clinical Pharmacist 1 position, both at Baylor St. Luke’s. In 2016, Shane started as a Clinical Pharmacist Specialist in Ambulatory Care at the Memorial Hermann Medication Therapy and Wellness Clinic in the Texas Medical Center, where he managed patients’ medication for anticoagulation and diabetes under collaborative practice agreements. While at Memorial Hermann, Shane became the DOAC protocol lead and developed close relationships with many patients. Shane returned to UHCOP in 2018 as the Director of Ambulatory Care APPEs. Since joining the UHCOP faculty, Shane has helped expand rotation offerings and diversity, coordinated the Pharmacy Law course, created the Transplant Pharmacy elective, and taken part in many committees within the college and AACP.
